1. Preparing for surgery

For best results from your surgery, it is important that you prepare for the surgery both PHYSICALLY and MENTALLY. It is no good embarking upon surgery while you are going through any significant life stresses such as moving house, starting a new job or getting a divorce! Please discuss with Miss Gupta / Professor Manyonda any personal matters that could impact on your surgery.

Here are some specific points:

As a general rule, the healthier you are prior to surgery the quicker you will recover. Eat sensibly and exercise regularly for at least a couple of months ahead of your surgery.

Smoking is a no no: it will affect wound healing and skin quality, and compounds the risks of any general anaesthesia. At the Haumea Clinic we strongly encourage you to give up smoking altogether. We recognize that giving up can prove very difficult, and the compromise we suggest is that you refrain from smoking for the two weeks prior to surgery, and the first six weeks after.

Time off work:
for most of the procedures performed at the Haumea Clinic, we strongly advise that you take a minimum one week, ideally two, off work so that you can be at home or in an environment where it is possible for you to attend to personal hygiene. The strict post-op instructions require that you have access to bathroom facilities where you will feel comfortable with attending to personal hygiene issues that are crucial to successful outcomes of your surgery.

Weight loss:
the vulva can change in appearance after massive weight loss – indeed that may be the reason that you are now seeking cosmetic genital surgery: a stable weight that you are happy with is essential to achieve good results that last.

The internet as an additional source of information: the internet can be a brilliant source of additional information on just about any topic you might wish to research, including cosmetic gynaecological surgery. However, there can also be some inaccurate and /or misleading information out there. Please discuss any issues with Miss Gupta / Professor Manyonda if you have any doubts. 2. The next steps:

Miss Gupta / Professor Manyonda will design a bespoke journey unique to you. Please always feel free to ask questions if there are any issues you are not clear about. No question is a stupid question if you need clarity.

Initial appointment:
At this first consultation Miss Gupta / Professor Manyonda will establish your specific requirements. A detailed medical history will be taken, and you will have a physical examination relevant to the treatment you seek. For example, you will have an examination to determine whether there is significant vaginal laxity / looseness if you are seeking surgical vaginal tightening, or your external genitalia will be carefully examined if you are seeking labiaplasty and/or hoodoplasty. Please note that you may be advised that the operation you seek is not appropriate for you, and the reasons carefully explained. For example, vaginal tightening surgery does not resolve problems related to difficulties with orgasm, and therefore you would not be offered such surgery. A woman under the age of 21 years may be advised to defer labiaplasty as the appearance of the vulva could change with maturation, sexual activity and childbirth. The bootom line is that each woman is unique, and any advice will be tailored to her individual needs.

Plan and quote:
The type of operation most suited to your needs will be fully discussed and explained by Professor Manyonda / Miss Gupta at your consultation. Always feel free to ask any questions so that you fully understand what is planned. Additional information is also available on the website. The prices for all the procedures are quoted on the website. For your specific procedure the price will be confirmed, and you will receive this in writing. Please note that all invoices should be settled prior to your operation.

Cooling off period:
Although you may have made up your mind about the operation you seek, we at Haumea Clinic insist upon a minimum TWO week cooling off period before gping ahead with your operation. This will give you time to reflect on whether this really is the operation you want. Even if you have already paid for your operation, you can decide at any time not to go ahead with the operation, and the only money you will not be refunded is the consultation fee.

Second Appointment:
It is of vital importance that you fully understand what you are about to embark upon. Therefore if, after the full consultation, you have issues that you would rather address on another face-to-face consultation, please do not hesitate to request such a consultation. Please bear in mind that any consultation fees will form part of the full surgery package if you go ahead with the surgery. However, consultation fees will not be refunded if you do not go ahead with the surgery.

Pre-operative preparation:
You will be given advice on how to prepare for surgery. Some of this information will be in a written format, and you are strongly advised to read the material provided. However, any key / crucial information will be reiterated verbally.

The day of Surgery:
You will be given explicit, written information regarding where and what time to attend for your operation. Usually you will be advised to NOT eat or drink anything from mid-night prior to the day of your operation, although this might be modified if your operation is scheduled for an afternoon session. On the day of the operation your surgeon will again explain what the operation entails, its benefits and risks, and any additional procedures that might need to be undertaken. You will then be requested to sign a Consent Form for the operation. Before transfer to theatre you will also be seen by the Anaesthetist, who will explain all about the anaesthesia you will have, and allow you the opportunity to ask questions.

After your operation:
Immediately following your operation you will be taken to recovery where you will be closely monitored while your anaesthetic wears off. This will usually take a couple of hours. You will be assessed for any requirements for pain relief. Your surgeon will have administered a local anaesthetic that is designed to last up 8 hours, so that most patients will come round from the anaesthetic and feel no pain at all. However, you should not let this lull you into a sense of false security about your need for pain relief – you should take the pain killers that will be prescribed before you leave the hospital: do not allow pain to build up as you will then find yourself “chasing your tail”, so to speak.

Hospital discharge:
Following surgical vaginal tightening procedures, patients are usually kept overnight, and allowed home the following morning. For all other procedures, patients are discharged home on the same day. Prior to leaving the hospital you will be provided with clear written instructions on how to look after yourself, especially over the following couple of weeks after the operation. The importance of strict adherence to the post-operative instructions cannot be over-emphasised, especially the taking of pain killers, antibiotics and “icing” the vulva following labiaplasty / hoodoplasty, and keeping the area clean and dry.

Please note that you cannot go home alone by taxi following your operation: please make arrangements for someone to pick you up and drive you home / travel with you in a taxi.

Follow-up appointments:
You will be seen at 1 -2 weeks following your operation to ensure that your recovery is following the right course, and to reassure you. Many women are often surprised by how little pain they experience, and how quickly they heal from cosmetic genital surgery. However, the occasional woman is equally surprised by the pain she experiences, and / or worried by how swollen the vulva can become following the operation. Each woman is different, but for most the swelling significantly subsides within two weeks, while in a minority of women it can still be noticeable at 4-6 weeks.

You will be seen by your surgeon again at 6-8 weeks following your operation. While for the majority of women the second follow-up appointment is the last consultation needed, for the occasional patient a third follow-up appointment may be required. Patients are usually advised to avoid intercourse for 8 weeks following these procedures, and the second appointment therefore allows your surgeon to give you the green light to resume full normal activity. However, apart from intercourse, most other activities will have been resumed from around two weeks following the operation.

Long term results:
The long term results of most surgical procedures are often not immediately evident: this is certainly true of most of the procedures performed at the Haumea Clinic. It is certainly the case that the swelling that is often seen in the first few weeks can be a cause for concern following labiaplasty, and it is vitally important for patients to be aware that it does take time for the desired results to become evident. Your surgeons will advise accordingly. Local massage with a bio-oil after four weeks, and resumption of intercourse after 8 weeks, are part of the process of shaping the long term appearance.
